Verizon repair problem

I am new to this forum and wanted to see if I could find some help or information on how to reach the right management person at Verizon to complain about a repair problem at my business. We have 6 rotating lines and a week ago all lines were out. We had a service call and they spent most of the day here in our closet where all the wiring is and before they left we discovered that one phone jack was dead. We brought it to their attention and they said it must have not worked before. It has been working just fine for 15 years in that same spot. Since then we have called several times to get this fixed. This morning someone came out with a "supervisor" and they said the jack is dead and it must have been that way before. They gave us a bill for coming out this morning and will bill it on our regular bill. I have two problems: 1) I want the jack fixed back like it was before they came out and 2) I want them to credit this bill off my account. Since one of the people who came out was a supervisor and she does not believe us, I don't know who to contact at Verizon to get this taken care of. Does anyone have names or numbers of who to contact for this kind of problem?
